3、Qualification Requirements For Bidder
Qualifications or Performance:(1) Bidders must provide a valid business license and if the products they are bidding on belong to one of the following categories: Category 1: Provide the "First Class Medical Device Filing Certificate" and "First Class Medical Device Production Filing Certificate" for the products they are bidding on. Category 2: Provide the "Second Class Medical Device Business Filing Certificate" (excluding bidders who are production enterprises), and provide the "Medical Device Production License" (excluding imported equipment) and "Medical Device Registration Certificate" for the products being bid on. Category 3: Provide the "Medical Device Business License" (excluding bidders who are production enterprises), and provide the "Medical Device Production License" (excluding imported equipment) and "Medical Device Registration Certificate" for the products being bid on. Provide copies of corresponding category identification documents. Those that do not belong to medical device products do not need to be provided.
(2) The bidding products provided by the bidder must have legal source certification documents. If the bidding product is an imported product, ① if the bidder is not a manufacturer and the authorized power of attorney is entrusted to the bidder, a power of attorney issued by the manufacturer of the imported equipment of the bidding product must be provided; ② If the bidder is not a manufacturer and the authorized party of the power of attorney is not the bidder, the bidder shall provide a power of attorney issued by the manufacturer of the imported equipment for the bidding product, as well as supporting documents provided by the authorized party corresponding to the power of attorney (such as product distribution agreement, regional agency certificate, regional agency authorization). If the bidding product is a domestically produced product, proof of the source of the bidding product (such as sales contract, agency contract, etc.) must be provided.
